New Patients
At Dental Designs of Roseville, we want you to feel comfortable – even pampered. That is why we make every effort to treat our patients as guests.
We will review your health information, take necessary digital x-rays, complete a full comprehensive exam including oral cancer screening. After all necessary records have been gathered we will provide you with a treatment plan and cost estimate. At this time we can discuss your questions or concerns. Our goal is to educate you so that you’re able to make your own decisions about your dental care.
If you are a new patient please take a few minutes to fill out our new patient forms prior to your appointment. If you are unable to print these forms out please arrive to your appointment 30 minutes early so that you have time to fill out our new patient forms.
